Guidelines for Online Application
  1. Fill up all the requested information. If the requested information is not applicable in your case, please type N/A.
  2. Click the “Submit” button when you have completely filled up all the requested information. Your application will be automatically saved in our record of applicants. This is only the first phase of your application process.
  3. Be sure to note down and/or remember your reference number when you submit your online application. The reference number will serve as your access to revisit and edit your online application. Please make your final editing before submitting your printed application form.
  4. Ensure that you have filled up the requested information before printing. Once submitted, you can no longer edit your application form online.
  5. PLEASE PRINT YOUR ONLINE APPLICATION FORM, AFFIX YOUR SIGNATURE, AND SUBMIT IT PERSONALLY TO THE ADMISSIONS OFFICE TOGETHER WITH THE LISTED APPLICATION REQUIREMENTS.
    FOR GRADUATE SCHOOL, SUBMIT THE APPLICATION FORM AND OTHER ADMISSION REQUIREMENTS TO THE GRADUATE SCHOOL OFFICE. GRADUATE SCHOOL IS OPEN FROM TUESDAY TO SATURDAY 8:00AM-5:00PM.
  6. Additional instructions will be provided to you by the Admissions Office upon the submission of your signed application form.
